How do I check that a plumber is licensed in Utah?
Search the official register: Utah Division of Professional Licensing (DOPL), Department of Commerce publishes a public lookup at https://secure.utah.gov/llv/search/index.html. Ask for the licence number and confirm the name on it matches the company on your paperwork.
Who is responsible if the work goes wrong?
